- Commitment to Access Enrollment for 2008 - Medicare Part D Patients
To qualify for Commitment to Access in 2008, Medicare Part D enrollees will need to meet the following eligibility requirements:
- Submit proof of spend showing at least $600 out of pocket prescription expenses for 2008.
- Provide a copy of the Part D Plan card.
- Patients must be enrolled in Commitment to Access. The re-enrollment form and proof of income will be due at the patient’s re-enrollment date.
