Patients or advocates can complete
and print the application
online. Applications can also be requested by calling Commitment to Access at 1-8ONCOLOGY1.
The patient's advocate is required to initiate the application process by calling 1-8ONCOLOGY1.
Each application has a unique 9-digit identification number that serves as the patient's
Commitment to Access ID number for as long as the patient remains in the program. Because
the application has a unique ID number, it may not be photocopied.
