How Do I Enroll in Commitment to Access?

• All patients must be enrolled through an Advocate (ie, someone such as a health care worker, social worker, or case worker, who is involved in the delivery of the patient's healthcare).
• The Advocate will complete an application and then call Commitment to Access at 1-8ONCOLOGY1 (1-866-265-6491) to enroll the patient.

How Do I Know If I May be Eligible for Commitment to Access?
